Please follow registration procedures below.

 

1. If you’re interested in attending, fill out the blank registration form below. 

2. These fields are required to consider the application complete (Full Name as printed on any legal document, DOB, Complete Address, Phone #, and Signature is a MUST).  SS# is not required.  Incomplete registrations will not be processed.    

3. Please do not write inside the box concerning any payment, this course is FREE.

4. Once registration form is complete, please scan and email it to:nosegued@epcc.edu, fvazqu29@epcc.edu, and ggamboa6@epcc.edu

 

Please note, if anyone has a Business Hold with EPCC, their registration form will NOT be processed.   All applicants will be notified of the Business Hold. Once applicant resolves the hold, please confirm by following up with an email so that we can resubmit the form for processing.
